Long Term Health Conditions Service Mapping Across Mid and South Essex

Working in partnership with colleagues from the ICS we are keen to ensure physical activity pre-hab/rehab services don’t differ depending on an individual’s postcode or long term health condition. There is currently a funded cancer pathway called ‘Living Well with Cancer’ running across MSE and we are working towards a proposal to develop a model that includes more long term health conditions.

Prior to the development of the proposal we are undertaking an important mapping exercise. We are currently exploring the support that’s available to individuals living with Long Term Health Conditions via a healthcare or community service that has a direct referral pathway.
